How do I get my Death Certificate Apostille

You are required to have an Long form Death Certificate with a Letter of Exemplification in order for your Death Certificate to be Apostille.

The Long form Death Certificate is not issued during walk-in service and may take up to 2 to 5 week before it is mailed to you.

  1. Once you have your long form Death Certificate and Letter of Exemplification, send it to us along with your order form
  2. We will get your Apostille processed when it is received and have your Apostille back in 48 hours.

Please contact the NYC Department of Health & Mental Hygiene and request a new Death Certificate (Long Form) with an Letter of Exemplification:

If you want me to get your Death Certificate then you need to send:
  1. A notarized letter authorizing Alan Murray to receive your Death record. 
  2. The APPLICATION FOR A DEATH RECORD.
  3. Your letter indicating what you want me to do.
  4. A copy of your Passport (especially if your document is being used in another country) or other valid Photo Id

Note:

A valid Identification (ID) is one with your photograph & signature. ID must be issued by an officially recognized organization or agency and includes the following types of ID: Driver's License, Employment ID, Government ID, Social Services ID, and a Passport. ID must show an expiration date that has not passed. ID without an expiration date must have been issued within the past 12 months.

Please make sure the Signature on the notarized letter
matches the signature on photo ID.

How do I get my Birth Certificate Apostille

You are required to have an Long form Birth Certificate with a Letter of Exemplification in order for your Birth Certificate to be Apostille.

The Long form Birth or Death certificate is not issued during walk-in service it will be mailed to you and it may take up 5 week to receive it . You also have the option to do this my mail which will take much longer.

  1. Once you have your long form Birth Certificate and Letter of Exemplification, send it to us along with your order form
  2. We will get your Apostille processed when it is received and have your Apostille back in 48 hours.

To get a certified copy of your Birth Certificate, Please contact the NYC Department of Health & Mental Hygiene (see address below) and request for a new Birth Certificate (Long Form Only) with an Letter of Exemplification.

Is there a quicker way to get my Birth Certificate Apostille?
Yes, there is.
You can get your Birth Certificate Apostille in one day. However, there is no guarantee that the person who it is going to receive it will accept it. So you must check with them to see if this process is acceptable.

This shorter process require you to create a letter indicating that the attached document is an original document issues by the Department of Health & Mental Hygiene, Office of Vital Records. Once you sign the letter & have it notarized it can be Apostille.

MARRIAGE CERTIFICATE APOSTILLE

How do I get my Marriage Certificate Apostille?

NYC MARRIAGE CERTIFICATES

You are required to have an Extended (Long) form Marriage Certificate with the stamp of the City Clerk on the back of your certificate in order for your Marriage Certificate to be Apostille. The Extended form has additional information on it (i.e. the ministers name, previously marriages, etc) that is not on the short form.

If you want NYNOTARY4U to get your Extended Form Marriage Certificate then we need to have the following documents:
  1. A notarized letter authorizing Alan Murray to receive your Marriage  record.  *
  2. The Draft copy of the APPLICATION FOR A MARRIAGE RECORD or a copy of your marriage certificate that you want to get Apostille.
  3. Your letter indicating what you want me to do.
  4. A copy of your Drivers License or Passport (especially if your document is being used in another country) or other valid Photo Id.

Note:

A valid Identification (ID) is one with your photograph & signature. ID must be issued by an officially recognized organization or agency and includes the following types of ID: Driver's License, Employment ID, Government ID, Social Services ID, and a Passport. The marriage License bureau will refuse to honor any request if there is a question about the signatures not matching.

* If you are outside of the USA make sure you get your document notarize at the US Embassy otherwise you will need to get an Apostille on your letter to me.
